Acceptance of These Terms:

By accessing or using this website (the “Site”), you agree to these Terms & Conditions (“Terms”). If you do not agree, do not use the Site.


Clarity Integrated Wellness (“Clarity IW” or “Organization” or “We” or “Us”) respects your privacy and is committed to protecting it through our compliance with this policy. Clarity IW provides outpatient mental health services for adolescents, individuals, and couples (collectively, “Services”).

 

Purpose of the Site (Information Only):

The Site provides general information about our services, policies, scheduling options, and educational resources. Nothing on this Site is medical, psychological, or legal advice, and it is not a substitute for professional care.

 

No Emergency Services:

The Site is not designed for crisis support or emergencies.

If you are in immediate danger or experiencing a mental health emergency, call 911 (U.S.) or go to your nearest emergency room.


U.S. crisis resources:

  • 988 Suicide & Crisis Lifeline: Call/Text 988.

  • If outside the U.S., contact your local emergency number or crisis hotline.

 

No Therapist-Client Relationship Created by Website Use:

Using this Site, sending a message through the Site, or requesting an appointment does not create a therapist–client relationship. A therapist–client relationship begins only after:

  • an appointment is scheduled,

  • informed consent and required intake paperwork are completed, and

  • the Practice confirms services are appropriate and accepted.


Communications and Contact Forms:

If you submit information through a contact form, email, or other messaging feature:

  • You understand that standard email and web forms may not be fully secure.

  • Please do not send sensitive personal health information through non-secure channels unless explicitly instructed.

  • We may respond using the contact method you provided, unless you request otherwise.


If you use the Clarity Integrated Wellness, LLC (“Clarity IW” or the “Practice”) secure client portal (if available), that portal’s terms and privacy practices also apply.  


Scheduling, Cancellations, and No-Shows (If Applicable):

  • Appointment requests are not confirmed until you receive confirmation from us.

  • Cancellation/reschedule policy requires 24 hours’ notice.

  • Late cancellations/no-shows fees: $225/individual session, $250/couples or family session, $150/group session/participant, with a sliding scale possible based on Clarity IW’s previous agreement.

  • Late arrival policy relates to clients being more than ten (10) minutes late.  All sessions end at the scheduled time.  


Fees and Payments

Fees may change at any time. The Practice requires payment methods on file and charge according to our policies. Insurance is not accepted.  Consequently, eligibility/coverage is determined by the payer and is not guaranteed.  A Superbill will be provided to clients upon request.  Outstanding balances are to be paid prior to Services.  A Sliding Scale is available at the discretion of the clinician.  


Telehealth:

  • Telehealth may involve specific benefits and risks (e.g., technology failures and/or privacy considerations).

  • Clients are responsible for being in a private location and using a secure connection where possible.

  • Psychotherapy telehealth availability may be limited by licensure laws and a client’s physical location at the time of service.


Age Requirements:

This Site is intended for individuals who are at least 18 years old, unless a parent/guardian is engaging for services for a minor as permitted by law and practice policy. If you are under the age of majority, do not use this Site without parent/guardian involvement, where required.


Privacy and Protected Health Information (“PHI”):

Your use of the Site is subject to our Privacy Policy. These documents explain how we handle personal information and, where relevant, protected health information.

Intellectual Property:

All content on the Site (text, logos, graphics, videos, downloads, and design) is owned by or licensed to the Practice and protected by intellectual property laws. You may view, download, and print content for personal, non-commercial use only. You may not reproduce, distribute, modify, or create derivative works without written permission. 


Acceptable Use:

You agree not to:

  • Use the Site unlawfully or in a way that could harm others;

  • Attempt to access restricted areas, accounts, or systems without authorization;

  • Introduce malware, scrape content, or disrupt Site functionality;

  • Misrepresent your identity or submit false information.

Third-Party Links and Tools:

The Site has links to third-party websites and uses third-party tools (e.g., scheduling platforms, payment processors, video platforms). We do not control and are not responsible for third-party content, policies, or practices. Use them at your own risk and review their terms/privacy policies. 

Disclaimers:

The Site and its content are provided on an “as is” basis without warranties of any kind. While we strive for accuracy, we do not guarantee that the Site will be error-free, uninterrupted, or that information will always be current.

Limitation of Liability:

To the maximum extent permitted by law, the Practice will not be liable for any indirect, incidental, special, consequential, or punitive damages arising from or related to your use of (or inability to use) the Site.

Indemnification:

You agree to indemnify and hold harmless the Practice from claims, liabilities, damages, losses, and expenses (including reasonable attorneys’ fees) arising out of your misuse of the Site or violation of these Terms.

Changes to these Terms:

We may update these Terms at any time. The “Last updated” date reflects the most recent changes. Continued use of the Site after changes means you accept the updated Terms.

Governing Law:

These Terms are governed by the laws of the State of Texas, without regard to conflict-of-law principles. Venue for disputes will be in Harris County, Texas, unless otherwise required by law.
